Optimum RTS is currently seeking a qualified Registered Nurse (RN) for a Home Health / Private Duty Case Manager role with one of our valued clients a leading provider of compassionate home healthcare services based in Vero Beach, Florida.
About the Role:
This is a hybrid office and field-based position offering the opportunity to play a pivotal role in patient care management. The ideal candidate will have experience in both Home Health and Private Duty Nursing, and will manage a current census of approximately 45 patients, with the potential for growth.
Key Responsibilities:

Conduct patient admissions and develop individualized care plans

Perform supervisory visits and caregiver clinical competency evaluations

Oversee coordination of medical and support services

Collaborate with families and multidisciplinary care teams to ensure high-quality, patient-centered care
Qualifications:

Active RN license in the state of Florida

Previous experience in both home health and private duty settings required

Excellent clinical judgment and communication skills

Strong organizational skills and ability to manage a dynamic caseload
